What To Expect on Your First Visit

What do I need to bring to my first appointment?

If we are billing your insurance, please bring your doctor’s referral, your insurance card(s) and a photo ID. Wear comfortable clothes that will allow your therapist to access your injured area(s). Please arrive 10 minutes early for us to process your paperwork and get you back to your therapist on time. You can also complete the forms from the links above and fill them out at home (strongly preferred).

Your First Visit Will be Scheduled For a Full Hour so We Can go Through The Following:

A thorough History, Examination, and Movement Assessment

A complete patient interview is conducted to determine your mechanism of injury, medical history, rehabilitative needs and goals. Then, a comprehensive functional postural & movement assessment will be completed to better understand the source of your pain

Understand your Condition and Plan of Care

A mechanical diagnosis is developed incorporating your goals, the pain source and the causative and contributing factors that are leading to your problem. Patient education is provided on understanding your conditon, the expected outcomes, and length of rehab needed, as well as a good understanding for what you, as the patient, can do to maximize gains. Your individual goals are devleoped with you as the focus. With your input, we will work to develop a custom plan that is specifically designed to meet your needs.

Treatment

You will receive treatment on the first visit from experienced therapists with advanced and effective techniques. Make sure to come prepared in athletic attire. Following the therapeutic intervention you will be reassessed to examine your gains.

Treatment is more than just the “stretching” people think of when they hear “physical therapy.” We use functional corrective exercise techniques, dry needling, instrument assisted soft tissue manipulation, taping, and other therapeutic techniques to improve the quality and/or quantity of movement.
